Community Connections Partnership

Community Connections Partnership (CCP) is a non-profit organization in the Minneapolis/St. Paul metro area providing support to people with disabilities. We specialize in the areas of employment, life skill development, healthy lifestyles, community supports, and recreation services. Our dedicated staff are committed to uncovering each person’s potential and supporting them step by step in their growth and development. We work in a spirit of partnership with the people we serve and their team members to achieve positive results.


Who Uses Our Services?

All of Community Connection Partnership services are open to adults with intellectual and developmental disabilities, including those with autism, Asperger's syndrome, cerebral palsy, and epilepsy. Many of the people we serve also need support with mental health challenges such as depression, anxiety, and mood disorders, as well as memory impairment, dementia, and chronic diseases. No matter what a person’s diagnosis is, we believe everyone has something to contribute and seek to create a supportive environment where this can happen. 

Funding Sources: Our services are available to people living in the Twin Cities metro area who receive funding through private pay, long-term care insurance, Medical Assistance, Home and Community Based Waivers including Brain Injury (BI), Community Alternatives for Disabled Individuals (CADI), Developmental Disabilities (DD), and the Elderly Waiver (EW).
